This card contains improvements from our CTC-6 Travel Card allowing for recording patient contact information and insurance data. The new format reduces insurance rejection rates when properly completed. Two-sided card allows you to record 12 visits/treatments. Printed on 8.5" x 11" white card stock for added durability. There is a 15% restocking fee on all returns.
